The following Psychiatric Advance Directive form is the product of extensive study and stakeholder consultation by a team at the University of Virgina Medical School. Dr. Richard Bonnie (University of Virginia), John Oliver Esquire (University of Virginia) and Dr. Jeffrey Swanson (Duke University) have been working collaboratively to study the role of Psychiatric Advance Directives in mental healthy care.
